Deer Park Families:
Based on the changes to student schedules across the district, we have made the determination to re-open the form allowing families to sign up for lunch distribution each Sunday.
The lunch packs include breakfast and lunch based on whether the student is in-person hybrid or fully remote learning. The package details are listed below.
We are asking that ALL families in need of lunches complete the form, even if you have previously completed a request, so that we can get an updated and accurate count for our distribution.
Please fill out the form below if you would like your student to receive these breakfast-lunch packs. If you have multiple children, please fill out a separate form for each child. It is very important that these forms be completed so we are aware of how many lunches we need to prepare each Sunday.
In-Person Hybrid: Packs of 5 breakfasts and 3 lunches will be distributed each Sunday from the back cafeteria entrance of Amity Elementary 1-2 p.m. and will contain breakfast and lunch for the following week.
Remote: Packs of 5 breakfasts and 5 lunches will be distributed each Sunday from the back cafeteria entrance of Amity Elementary 1-2 p.m. and will contain breakfast and lunch for the following week.
Kindergarten: Packs of 5 breakfasts and 5 lunches will be distributed each Sunday from the back cafeteria entrance of Amity Elementary 1-2 p.m. and will contain breakfast and lunch for the following week.
We will also hold an extra pickup each week on Monday afternoon from 3:15-4:15 p.m. for those unable to pick up on Sunday.
ALL students can pick up packs, whether elementary or jr./sr. high school level, however, all packs will be distributed from the back entrance of the Amity cafeteria.
