In accordance with guidelines set by Hamilton County Public Health Department, a student who is experiencing one or more Category 1 symptoms or any combination of two or more Category 2 symptoms is required to self-isolate for 10 days unless the student is evaluated by a physician and it is determined that their illness is not COVID-19 related and may return to school.
A negative COVID-19 test alone will not be accepted for your child to return to school. In order for a student to return to school, the Return To School Form must be completed by your child’s physician.
Please be prepared to take this form with you to your child’s appointment and to have it completed by his/her/their physician. Completed forms must be turned into the school nurse prior to your student’s return to school.
Forms may be emailed to medical@dpccsd.org or faxed to the Jr./Sr. High School (513-891-3845) or Amity Elementary (513-891-3508).
Please contact your school’s nurse if you have any questions or concerns.
Sincerely,
Dr. Orso
Assistant Superintendent
